Older People and Physical Disabilities Assessment and
Care Management Community Teams
With effect from 16 January 2006 we have reorganised the above teams into the following six
teams covering the social care needs of older people and younger adults with physical disabilities,
including people requiring Social Work or Occupational Therapy services:
North West Team (based at Oswestry)
North East Team (Wem)
North Shrewsbury Team
South Shrewsbury Team
South West Team (Craven Arms)
South East Team (Bridgnorth)
Countywide specialist Sensory Impairment Services (Rehabilitation Officers for people with visual
impairments and Technical Officers for people with hearing impairments) will continue to be provided
from St Michael’s House, Shrewsbury, but social work services are devolved to the local teams above.
The Royal Shrewsbury Hospital Joint Interface Team is unchanged.
